• Development of the person centered service plan that address Participant identified areas of need, goals, intervention, and progress of goals
• Coordinate services for Participants in relation to their identified needs
• Monitor authorized services through phone calls, face-to-face visits, person centered planning team meetings, and contact with providersThe Service Coordinator position can be performed remotely and will require travel within Lancaster County.
• Must have a Bachelor’s degree in social work, human services, psychology, sociology, anthropology, gerontology, behavioral health with practicum experience or in lieu of a Bachelor’s degree have at least three or more years of experience in a social service or healthcare related setting.
