We believe in whole health. This means we address and heal disease but equally important, we work at the causes of the causes, the social ills that must be addressed to achieve true equity.

Union Community Care is a nationally recognized Patient-Centered Medical Home because of our inclusive approach to providing healthcare services that reflect the cultures, values, and needs of the individuals we serve. Our progressive care teams care deeply for their patients and come to our community with unique experiences and diverse cultural backgrounds.
Our Board of Directors
As a 501(c)3 Community Benefit Organization, 51% of our volunteer Board of Directors are patients of Union Community Care. The result is a combination of community leaders who understand and embrace complex lives and unique strengths and work hard to break down all barriers to care.
Our network of care
We provide primary family medical care, dental care, behavioral health, and social support throughout 10 Lancaster & Lebanon locations. Our Sliding Fee Discount Program offers a discounted rate or nominal fee for medical and preventive dental services offered at our centers based on household income and size.
